In a new Instagram video, Dr. Pimple Popper—aka, dermatologist and TLC host Dr. Sandra Lee, MD—pops a sizable lipoma out of a patient’s forehead. Yes, that’s right. Dr. Lee removes a bulging lipoma from the forehead of a patient who is only under local anesthesia. Yikes.

In the clip, Dr. Lee makes an incision into the patient’s forehead and starts fishing out the lipoma in pieces — all while the patient is awake and speaking to her.

“Come on, little friend,” Dr. Lee sings to the lipoma as she tries to coax it out little by little.

Dr. Lee tells the patient that lipomas on the head tend to adhere to internal structures, making them extra tricky to remove. This patient deals with a bit of lipoma tug-of-war, but not nearly as bad as Dr. Lee expected.

“I was actually really please because yours popped out to some degree,” Dr. Lee tells the patient.

As always, fans had creative ideas of what the lipoma looked like. One especially creative soul was inspired by the final piece to exit the patient’s head, commenting, “Oysters on the half shell? Anyone? Anyone?” No thanks, bud.

A lipoma is a fatty noncancerous growth that grows between the skin and muscle, and is usually a yellowish-orange color.
